Abstract

A 45 year-old man was admitted due to early recurrent parasagittal meningioma with malignant transformation 22 months
after initial total tumor resection. The initial surgical specimen was diagnosed as meningotheliomatous meningioma which
had no malignant features. Brain magnetic resonance imaging(MRI) showed a recurrent tumor located at the same area of
the prior surgery. After second operation, the tumor was pathologically compatible to atypical meningioma. It is rare for a
benign meningioma to recur with malignant transformation shortly after surgery. Surgeon should keep in mind that benign
meningioma can transform to aggressive tumor in short term and intermittent imaging follow up is needed.
